WCAG 2.2 success criterion 1.1.1 requires a text alternative for non-text content. In practice the two failures worth hunting are an image with no alt attribute at all, and an image whose alt text is a filename like IMG_4021.jpg, which describes nothing.
Note the distinction that trips people up: alt="" is correct for a purely decorative image. It tells a screen reader to skip it. A missing attribute is the bug; an empty one is often the right answer. This scanner treats them differently, as it should.
